* breakpoint.c (delete_breakpoint, enable_breakpoint,
	disable_breakpoint): Don't call breakpoints_changed; it is now
	called via the *_breakpoint_hook functions.
	* annotate.c (_initialize_annotate, breakpoint_changed): New functions.
