convert to_attach

	* target-delegates.c: Rebuild.
	* target.c (init_dummy_target): Don't initialize to_attach.
	(target_attach): Unconditionally delegate.
	* target.h (struct target_ops) <to_attach>: Use
	TARGET_DEFAULT_FUNC.
