convert to_terminal_init

	* target-delegates.c: Rebuild.
	* target.c (update_current_target): Don't inherit or default
	to_terminal_init.
	* target.h (struct target_ops) <to_terminal_init>: Use
	TARGET_DEFAULT_IGNORE.
