* monitor/monitor.h: Remove STREQ, STNEQ (pollute namespace),
	RTEMS_MONITOR_PROMPT (pollutes namespace, useless).
	* monitor/mon-command.c: Add RTEMS_MONITOR_PROMPT.
	* monitor/mon-monitor.c: Apply private version of STREQ.
